Things to bring to your appointment:

1. New Patient Forms above (if you haven't already mailed them)
2. Insurance Card(s)
3. Photo ID
4. Co-Payment (if applicable)
5. Recent X-rays and MRI's (if not taken at The Christ Hospital)
